• We are happy to verify general eligibility and submit claims to your insurance. However, insurance companies determine how benefits apply, and patients are responsible for confirming their deductible and coverage details with their insurance plan.

• You will receive information on how to enter the building in your appointment confirmation and reminder emails.

• We require a card on file at the time of booking. There is a $50 cancellation fee for all no-shows and cancellations with less than 24 hour notice.
