Insurance information

• We verify general eligibility with 2 business days notice as a courtesy and submit claims to your insurance. However, insurance companies determine how benefits apply, and patients are ultimately responsible for confirming and understanding their deductible and coverage details with their insurance plan.

• We are in network with: Regence, Blue Cross Blue Shield, Anthem, UMR, United, Aetna, Pacific Source and Moda. We are not in-network with Medicare Advantage plans associated with these insurers.

• Massage services are out of pocket only.

Clinic + appointment information

• You will receive information on how to enter the building in your appointment confirmation and reminder emails.

• We require a card on file at the time of booking. There is a $50 cancellation fee for all no-shows and cancellations with less than 24 hour notice.
